Opportunity for GP Practices to access funding to support improved self-management of CVD

Do you have ideas about how to improve Cardiovascular Disease care for your patients? If so, we would like to hear from you! The South Yorkshire and Bassetlaw Integrated Care System (SYB ICS) and the Yorkshire and Humber Academic Health Science Network (AHSN) have partnered to run an Exemplar Innovation Programme and we’d like your help.

The Exemplar Innovation Programme provides an opportunity for GP practices to apply for funding to develop a pre-existing or new solution aimed at improving self-management within Cardiovascular Disease (CVD).

You can either apply individually or collaborate with multiple practices. To apply, simply fill in the two-page, ‘expression of interest’ form.

There will be a maximum of four projects awarded funding under the Exemplar Innovation Programme. Each project will be able to access a maximum funding allowance of £22,500

Support will be made available to help scope and plan the project and evaluate the impact. Delivery of the project will be the responsibility of the GP practice(s)
